How hard is it to swap over from another provider?

Its not! It's just a matter of contacting your current provider and asking for a cease date (usually a fortnight from your phone call), and then you call My Aged Care, and ask for a "referral code" as you are transferring over to Lifebridge. You then call us and we take care of the rest!

What services do you provide?

We provide Domestic Assistance, Gardening Services, Group Programs, Personal Care, Social Support Independence i.e. shopping, Transport, Respite, Carer Programs

Do you require an Aged Care Plan or Commonwealth Home Support Program?

Yes, but if you don't have one, don't worry, we can help you through the process.
